Complete Contract and Documentation

Complete and return the Assurance Screening and Solutions LLC Family of Companies Collection Site Management Service Agreement, along with current training certificates, calibration logs (if applicable), W9, and insurance coverage via email.

Getting Started

Complete your profile on the asapscreens App by entering your information, the types of drug and alcohol tests for which you are trained and qualified to conduct, and ensuring your notification tab is turned on when you are available. Finish the Collector registration, click on the “Register” button. asapscreens App Administrators will review your submission for approval.

Collector Responsibilities, Compliance, and Confidentiality Guidelines:

Responsibilities and Compliance:
  • The Mobile Collector’s responsibilities include but are not limited to collecting and preparing specimens for shipment according to local, state, and federal regulations. For DOT collections, the Collector must follow DOT regulations 40 CFR Part 40 and be a qualified Collector. For non-DOT collections, industry standards and guidelines outlined in DOT regulations 49 CFR Part 40 must be followed. The Collector also agrees to provide training documentation upon request from the C/TPA and be prepared to demonstrate proficiency.
  • DOT regulations require Collectors to be knowledgeable about 49 CFR Part 40, “DOT Urine Specimen Collection Procedures Guidelines,” and relevant DOT agency regulations. They must stay updated on changes and subscribe to OPADC’s list serve notices. Collectors must also receive training on proper collection procedures, handling “problem” collections, identifying flaws, and maintaining collection process integrity. After qualification training, Collectors must demonstrate proficiency through five consecutive error-free mock collections, covering different scenarios. This ensures competence in handling quantity discrepancies, tampering attempts, and employee refusals.
  • Dress professionally, act ethically, complete paperwork promptly, and send it to MRO, asapscreens, and the Company within 24 hours.
  • Commit to keeping training certificates, calibrations, and insurance current, supplying them to asapscreens upon request or expiration.

Confidentiality

  • Any information related to asapscreens APP users whom Collector provides products and services is confidential. Collector shall not disclose such information or use it, directly or indirectly, for any purpose outside the scope of this Agreement, including but not limited to solicitation of business from those customers.
  • The Collector will ensure that they keep records in a confidential manner and further agrees to maintain and store all required records in a confidential manner. This refers to and is not limited to handling, storing, transporting, or transmitting records, results, or information for asapscreens App or our Family of Companies. Please refrain from photographing, recording or video recording confidential information and most importantly without donor or company consent.
